To enroll students in grades K-12 who are new to Southeast Polk, the parent or guardian must contact the registrar to schedule an appointment to complete the registration process in person. Please contact Sue Howard at the District Office to set up an appointment.

A registration form must be completed by the student’s parent or legal guardian in order to enroll your student at Southeast Polk. Proof of legal guardianship needs to be provided in order to enroll the student. Please bring the following information with you to your appointment.
Custody Papers
If custody is of other means than divorce, the Guardianship and Custodial papers must have an assignment of tuition. This is handled through the courts, so please contact your attorney for assistance.
Proof of Residency
If you are new to the Southeast Polk school district, please be prepared to provide proof of residency showing your name and district address in any of the following forms. Please provide the appropriate documentation to the registrar prior to your student’s first day of school at Southeast Polk. Failure to do so may delay your student’s enrollment at Southeast Polk.
If you own property in the Southeast Polk school district, proof of residency for homeowners can be verified by the Polk County Assessor’s page. If homeownership is too current to be reflected on the Polk County Assessor’s page, you may be asked to provide one or more of the following:
- Homeowner’s purchase agreement
- Statement from your mortgage company
- Current utility bill mailed to you at your residence
- Any other bill mailed to you at your residence
- A cancelled check in your name showing district address
If you rent property in the Southeast Polk school district you will need to provide one or more of the following:
- Copy of the lease/rental agreement
- Current utility bill mailed to you at your residence
- A cancelled check in your name for the current month showing district address
- A pay stub from your current employer showing district address
If you are sharing a home or apartment with another individual or family you will need to provide one or more of the following:
- A statement from landlord or apartment manager indicating awareness of your living arrangements
- Current utility bill or any other bill mailed to you at your residence
- A cancelled check in your name for the current month showing district address
- A pay stub from your current employer showing district address
Falsification of any information or document required for residency verification or the use of the address of another person without actually residing there may result in:
- Revocation of student enrollment
- Being held liable to reimburse the district for expenses incurred to educate this student
- Civil action resulting from fraud, negligent misrepresentation and negligence
